Abstract
Therapeutic Drung Monitoring subcommittee was initiated in 1995 and two trials were performed during the first year. A total of four specimens covering 26 test items were distrituted. Three specimens were distributed to 59 laboratories with a
repsonse
of 85% in the first trial and one specimen to 68 laboratories with a response of 85% in the second trial. The participating laboratory number among the whole laboratories, 393, was 68 laboratories(17%). Total twenty six test items including
acetaminophen, amikacin, amitriptyline, carbamazepine, chloramphenicol, cyclosporine, digoxin, disopyamide, ethosuximide, free phenytoin, gentamicin, lidocaine, lithium, methotrexate, N-acetyl-procainamide, phenbarbital, phenytoin, primidone,
procainamide, propranolol, quinidine, salicylate, theophylline, tobramycin, valproic acid, vancomycin, were evaluated. The average test item number per laboratory is around seven and the most common test items were digoxin, phenytoin,
carbamazepine,
phenobarbital, theophylline, valproic acid and lithium in order, TDx drug monitoring analyzer (Abbott, USA) is used more than 90% among the participating laboratories. The other instruments are Stratus(Baxter, USA), Aca(Dupont, USA),
Solaris(Behring,
Germany), Axsym (Abbott, USA) and so on. The instrument group-related means of each item were used as designated target value, respectively. The standard of performance on the measurement the drug level was variable from 27.3% of lidocaine to
4.9%
of
procainamide. The results show something to be improved and the first year's results would serve as a baseline of the quality control score in the therapeutic drug level monitoring.
